- Referral letter for retirement from the employer (if he is on the job) and to whom it may concern from the General Organization for Social Insurance stating the length of service, and in case of settlement (a letter to whom it may concern) from the General Organization for Social Insurance stating the previous service period.
- Copy of the civil ID for the disabled.
- Copy of the Disability ID.
