- Civil ID’s for both people with disabilities and the assignment applicant (provided that the address in both cards matches)
- Birth certificate of the person with disabilities and the assignment requests.
- Disability identifies.
- Official notification (statement of the marital status of the assignment applicant and the person with a disability (18 and above).
- A certificate issued by a government hospital and a psychiatric hospital, stating that the assignment applicant is healthy.
- Criminal status sheet: There are no judicial rulings in a crime against honor and honesty for the assignment applicant.
- A certificate that there are no final judgments to quarantine a person with a disability from the Ministry of Justice (judicial inquiry) for people with mental and psychological disabilities (for ages 21 and above).
- Ruling on proving final custody of a person with a disability in the event of separation between father and mother.
- Sign the declarations prepared for this purpose.
In the case of submitting an assignment to a person with a disability who is incomplete or incompetent
- Bring the previous requests, except for a certificate that there are no final judgments to quarantine the person with disabilities from the ministry of (Justice and judicate inquiry).
- Submitting a medical report stating the incapacity of those who have priority according to the text of law article 25, if any.
- Submitting a death certificate for those who have priority according to the text of law article 25, if any.
